Student Information Forms

All student information forms sent home at the beginning of September must be completed and returned to the school.  It is very important that we have all the required information on file for your child.  Please place the completed forms back in the white envelope and drop it into the bin located outside the main entrance doors between 8:00 am and 4:00 pm.  If you have already returned your forms, thank you, and please disregard this message.
