Sports Physical Information for Student Athletes (Grades 6-8)

To participate in school sports, students in grades 6-8 must have a completed Preparticipation
Physical Form
(Grades 5-8) signed by their doctor. The physical is valid for one year from the
date of the exam. Please submit the completed form as soon as possible to ensure eligibility for
sports participation throughout the school year. Please see the ECS Athletics page for the
Preparticipation Physical Form deadlines for the current school year.

Timing of Physicals: The best time to schedule a physical is during the summer, as it will cover
all sports seasons throughout the school year. If the physical form is submitted and processed,
the student will be eligible to participate in sports, even if the decision to join a sport is made
later in the year.

New Process for Physical Forms:

  • The History Form (completed by the parent) and the Physical Examination Form (completed by the doctor) will remain in the student’s medical file at the doctor’s office.
  • Only the Medical Eligibility Form, completed by the doctor, needs to be submitted to the school.
  • After the Medical Eligibility Form is submitted, the school nurse will review it and send it to the school physician for final approval.
  • Once the school physician signs the form, parents will receive a copy of the signed form for their records.

Additional Required Forms:

  • Sport-Related Acknowledgement Forms (Concussion, Cardiac, Opioid) must be submitted every school year. These forms should be completed by both the parent and the athlete. They are required once per year.

Health History Update:

  • If a student’s sports physical was completed more than 90 days before the start of the sport season, a Health History Update Questionnaire (completed by the parent) is required and should be submitted to the school nurse’s office.
