IMMUNIZATIONS AND PHYSICALS
In accordance with New Jersey State Law, all students entering NJ schools are required to present documentation of having a physical examination and receiving proper immunizations. Physicals must be completed within 365 days prior to beginning school. Otherwise you will need to schedule a new exam and notify us of the appointment date. Please have your doctor provide a copy, or record exam results on the appropriate form. Note that if your student plans to join a school SPORTS team (6th-8th grade) within the next year, the exam MUST be documented on the NJ State Sports Physical form. Per the NJ Department of Health, the chart above lists only the vaccines that are required for school attendance in NJ. Consult with your pediatrician regarding additional vaccines that are recommended by the Centers for Disease Control and Prevention (CDC). Please note that unvaccinated children, including those with medical and/or religious exemptions, may be excluded from school during a vaccine preventable disease outbreak or threatened outbreak to ensure public health safety.
Each child attending must present documentation of immunizations or valid medical or religious exemption to vaccines. In order to allow a child to enter school, they must have at least one dose of each age-appropriate required vaccine. Of special note is that prior to begining 6th grade, students are required to receive two additional immunizations; a Tdap booster and meningococcal vaccine.
